Change of Information
Please note: Any new information submitted to the Office of the Registrar will become part of your permanent record at William Jewell College.
Change of Address: To change your address in our computer records, please print and complete the Change of Address form provided here, and send or deliver it to the Office of the Registrar. Or you may send written notice with the following information:
- Full Name (include all names under which you may have been registered at WJC)
- Social Security Number or Jewell ID number
- Current or Former Student (if former student, include last year attended)
- New Address and Phone Number; Email Address (optional)
- Former Address and Phone Number; Email Address (optional)
Change of Name: To change your name (i.e., marriage, divorce, adoption, etc.), please print and complete the Change of Name form provided here, include a copy of a government-issued photo ID (such as drivers' license, passport, etc.) indicating your legal name change, and mail or deliver it to the Office of the Registrar. Or you may send written notice with the following information (and enclose a copy of your photo ID):
- New Full Name
- Former Name (include all names under which you may have been registered at WJC)
- Social Security Number or Jewell ID number
- Current or Former Student (if former student, include last year attended)
- Current Address and Phone Number; Email Address (optional)
- Reason for Name Change (marriage, divorce, adoption, etc.)
- Type of legal document provided
Please note: Social Security Card will NOT suffice, since it is not a photo ID. We will no longer accept copies of marriage license, divorce decree, adoption papers, etc., as documentation for legal name change.
