We hope all is well and that you are looking forward to the next school year as much as we are. As usual, all incoming student athletes are required to have the proper medical and insurance information on file with the athletic training staff before participating. Before you may practice, the following information must be completed and submitted.
A Current Physical Examination
All physical examinations must be completed and signed by a licensed medical doctor (MD or DO). Physical examinations signed by any other medical provider other than those listed above will NOT be accepted. For clarification we will not accept physicals signed by nurse practitioners or chiropractors. -- Download Form --
Insurance Questionnaire and Copy of Card (front & back)
All athletes MUST have insurance coverage in order to participate. If you do not have insurance, we have information regarding temporary policies that may be purchased by the student athlete or you may contact G.M. Peters Agency, Inc. at 816-781-8050. Please include all information regarding insurance coverage, including secondary policies. -- Download Form --
Liability Waivers
